David, for the second part it just reads garbage after the first question. So if you were supposed to choose something before filling in the edit box you can't. Plus then it won't let you advance. 

I wrote the person noted and he said he would work on the problem. He asked what screen reader I used. I said the best, Window Eyes, grin. Just had to tease you, giggle.

Well, there is a bunch of extraneous info spoken with each radio button.  YOu can pull meaning out, but not convenient.\ \ Also, if you tab it doesn't behave well, arrow up and down, and use spacebar to make choice.
