Richard, 
I always appreciate your DVS TV info.  I would love a little info sheet that I could keep by the TV.  It is very confusing and I haven't made it work on our TV yet... but I have hope!

For films in theaters, http://www.captionfish.com is a good place to start.  You type in your location and it shows captioned and descriptive audio showings.  I have seen a dramatic increase of DVS showings in our local theaters.  I love to see adaptation and accessibility be the norm in all things, not just the exception.
