involvement in this research, but we believe that it may contribute to our
research mission.*



Hello,



My name is Katriana Belknap. I am a student at the State University of New
York at Brockport and a member of the Honors Program. I am currently
conducting research on physical activity and functional body image in
children with and without blindness or low vision under the supervision of
Dr. Melanie Perreault.



If you have a child who is blind or has low vision between the ages of 8
and 17 years old, I invite you and your child to participate in this study.
Participation will consist of your child completing an online questionnaire
(with your assistance) about physical activity, self-esteem, and functional
body image. This questionnaire will be completed two times separated by a
week. Upon completion of both questionnaires, your child will be entered
into a raffle to win one of five $50 gift cards. Total participation should
take approximately 40-60 minutes.
