Thank you all for the helpful information.  We figured it out since it ended up being a settings issue so the recet functions worked so, that's what she said when she called.

There are a few things to try.

1. It might just be that the volume is turned down.  What I do is flick left and right while turning up the volume.  Of course, he may not know if the phone is locked, so he'll want to perform this action a few times while pressing the lock button in between tries.
2. Rapidly press the lock button down about eight times, then wait about 30 seconds.  This performs a soft reset.
3. Hold the home and lock buttons down together for about eight seconds, then wait a couple of minutes.  This performs a hard reset.  No data is lost, but if voiceover was originally on, then it will come back on.
4. Plug the phone into the computer, bring up iTunes if it doesn't come up automatically, wait for the phone to show up under devices, find the summary tab, which is the first tab, then look for something like configure universal access button.  If voiceover is already turned on, I would turn it off, and then wait a bit and turn it back on.

Other tests I would do are: 1, call the phone, this will let him know that the battery hasn't simply run out, 2, Three finger double tap, which mutes and unmutes speach, and 3, tripple press of the home button which turns voiceover on and off.

I personally agree with you. The phone is a great device. I will let them know of these suggestions. For future reference, how do you turn on Voice Over in ITunes?

First, try tapping 2 times with three fingers. This should turn the speech
on if its turned on in settings. Second, plug the thing into itunes and set
voiceover options to speak. Don't be discouraged; the iphone is a great
product. When seeking technical support, you should look on the website and
email apple accessibility.

Does anyone know what to do when Voice Over shuts down on an IPhone and
sighted assistance is no ware to be found? I live alone. I use a Phone with
Voice Over. For two days I have had no luck communicating with apple to
resolve my tech support issues. The alarms keep going off and tech support
is not that helpful when calling. I have a full insurance policy with safe
ware communications. This is now day 3. Apple support has told me to do
things that require full vision such as checking for the arrows movement on
the screen. What can I do since the insurance company requires the phone to
be stolen to file a claim for a replacement? What do I do since the local
apple retail store is far away and requires a tech-support fee for turning
on Voice Over?
