As I said, A good noise cancelling USB headset will do the trick. I use xoom, Webex for court hearings daily with the ones I recommended and no jaws bleeding.

And one has a desktop computer, there are usb plug in mic available that allow you to use ear buds and have microphone on a desktop.
There are also webcams that work with usb that can be plugged into computer that have microphones.
