Hello Doug,
I'm not an expert in systems management but it looks to me that the error happened on line 477 which suggests a lot of work got done before the fatal error occurred.

Sorting out how packages are added on the fly is a miktex issue, not a tex4ht problem. I haven't been witness to an installation of miktex for so long that the memory is a little vague. You need to find the package manager, make sure it is operating using admin rights, and ensure that the mirror it is pointing towards actually does exist. I certainly do not use the one closest to me in geographic terms as it has proven unreliable in the past.

Getting this sorted out is important. If you end up processing the documents written by others, you'll soon find them using packages you don't have. Pulling packages into the installation is possible, but if you do, then you must update the package registry.

At about that point in the process you'll have to do what I do and seek guidance from someone in iT.

I don't know if this list accepts attachments so I copied the log file below.  Apparently htlatex isn't part of my installation so the package manager went out to find it and the attempt failed.  ---Doug Miron
