Emacs works under Windows 10 with a screen reader, and under Linux, of course, from the console. On the Mac, you need to run it in terminal mode rather than as a graphical application. Even then, it's awkward, as VoiceOver for Mac in the terminal isn't as refined an experience as one would prefer.

It's possible to use the Emacspeak spoken interface under Linux or Mac OS, however, which gives the best over-all accessibility for speech users. For braille output, I would recommend BRLTTY with the Linux console. NVDA and JAWS both offer good braille support in the Windows environment.
