[BlindMath] How to create the most accessible mathematical content?

Hi BlindMath Mates? Hope you're doing well.
How best to create PDF, HTML, or other documents with mathematical content that 

(1) are accessible using screen readers such as JAWS and NVDA 

and 

(2) can also be translated into UEB mathematics braille so as to be embossed using the Index Braille's Everest-D V4 or V5 braille embossers?
And, how best to create XML files with mathematical content with the above two properties?


Please describe the full step-by-step procedure in detail for a blind and visually impaired person.
