I haven't used Maxima myself on any platform, but my understanding is you need to use it in terminal mode. There was a post about this just the other day on the Blind Programming list

I’ve been exploring Maxima for a while. The gui seems mostly accessible, except when I want to interact with the input/output. Voiceover won’t read it, and there isn’t really something to interact with besides the toolbar. Is there a setting I need to enable?
