Perhaps an equivalent of the --self-contained option in Pandoc, which embeds all scripts and images into the HTML file, would be useful (but probably not as the default).
I've had good success with Lwarp and TeX4HT, which are compatible with the LaTeX packages that I tend to use. I use a makefile that builds both HTML and PDF versions of my documents from LaTeX source.
On the broader questions raised here, I use Markdown, HTML and LaTeX depending on circumstances, but I generally write my own work in LaTeX, as I typically need features that can be obtained from LateX packages but which Pandoc Markdown doesn't support. My current work does not involve writing mathematical notation, so my experience isn't relevant to the specifically notation-related aspects. I use Git for revision control.

These days TeX4ht, LaTeXML, and Lwarp are very easy to use.  In the case of math, is MathJax output sufficiently accessible?  What else could the tools provide that would help?

As an aside, a flaw that I see in MathJax is the reliance on a server, so I shall try to make Lwarp have a way to specify the server in an independent user-adjustable CSS file instead of having the selection be embedded in the HTML. Already I have seen a recommended server slow down at times with today's typical demand. Imagine if the documents using MathJax increased by an order of magnitude or two...
