[BlindMath] Reading Math with NVDA

If you are using MathPlayer, then it looks like it is using a pitch change
to indicate a capital letter. It is possible to change that to use a word
such as "cap" also or instead of the pitch change, but that is more
involved and requires changing the contents of a MathPlayer file (maybe
four lines or so). If you want to do that, let me know and I'll let you
know what file/lines to change.

If you are using MathCAT, it will follow the NVDA settings for "cap",
pitch, and beep set in the NVDA settings speech dialog.
