[BlindMath] Regarding NVDA with mathcat

Hi. This is not happening on a site. I have a book converted to accessible format in HTML using ISTEM portal which is no longer in working. The behaviour was experienced while I opened this file in google chrome. Now, I have opened another file which is converted to accessible format using mathpics; in that it is not happening. Perhaps it might be an issue with the file. However, in this file I am facing other issue: at times it says "error in reading math." When I try to navigate through the expression then it again says "error in navigating math." Any solutions for this are appreciated.
