You do not have to book your travel insurance through the cruise lines, any 
travel agent can sell it to you. they just need to know where and when you 
are going, and if you have pre-existing conditions, you had to have 
purchased it within 15 days of booking your cruise.

So you wouldn't get the same coverage through the cruise lines, they don't 
cover airfare and missed cruise because your plane was late or if you lost 
you job, etc.
