Dear R.J. and All,

There is a way that you can teach yourself how to write your name.
It takes a piece of wood and at engraving tool.
You need someone sighted to use the engraving tool to write your name into 
the wood so that a channel is dug into the wood that you can practice 
tracing with the tip of a ball point pen.
After you have traced the pattern of your name many times.  It builds muscle 
memory.  You will be able to write your name on paper.

This is only a suggestion, but I think it can work for you.
I write from memory all of the time and I can not see anything now.
People say that I write very well and they are amazed that I can write 
without a signature guide.
I can also write notes and other things in both block print and cursive 
forms and other people can read it.

You might be able to take a piece of wood to a jewelry shop, where they do 
engraving, and have them do it for you.
They should write it about the same size as you would if you could see and 
write it normally on something like the signature line on a check or form.
