This story reminds me of a incident back when I first started working for 
G.E. Nuclear Energy Division.
One of the techs was given the task of lighting off a gas fired furnace that 
was used to simulate the reactor core on our test rig.
He turned on the gas and after a minute started the blower that supplies the 
burners with air.
He then tried to light the burner, but for some reason it did not light 
right away.
Instead of turning off the gas, while he checked for the problem, he left it 
on.
When he tried to light it the second time, there was a explosion and the 80 
foot tall smoke stack on top of the furnace made like the space shuttle and 
flew 200 feet into the air and came down next to the parking lot about 500 
feet away from the building.  We all went running to see if we could help in 
case there was a fire in the furnace shed and to see if anyone was hurt.
When we got there this guy Sal greets us at the door,looking like Uncle 
Fester from the Munsters, bald headed, no eye brows , clothes smoking his 
face all black with only his eyes showing through.  He has this sheepish 
grin on his face and he is stuttering to say what happened.
He forgot to purge the cell before trying to light the burner the second 
time and the gas built up to the point that when it did light the explosion 
was too much for the cell.
Lucky for him, no one was hurt and his flash burns were minor.
>From then on he was just called Uncle Fester by everyone.
My first job that day was figuring out how to replace the smoke stack back 
on the furnace and making repairs to it.
This story just shows how easy it is to do something wrong .  This guy had 
lit this furnace over 100 times in the past without a problem, but admitted 
that he was lost in thought and thinking about something else when the 
accident happened.  the lesson is, "keep your mind on what you are doing or 
what you are doing can come down on your head."
