[Blindtlk] Looking for a CPA who can answer an IRA Withdraw Question

Good morning all:

 

I am looking for someone who is a CPA, that can contact me off list, or this
might be a question for those seeking the same answer.  I have an IRA
traditional and a Roth IRA, I have to use  due to bills and other issues.

 

It states that person's with permanent disabilities are exempt from
penalties. 

 

What is needed for 2016 to show the IRS this, do I attach it with my tax
forms, etc.
