I have not found a definitive source on this yet, but frankly, there are some differences among note-takers so there not be a truly definitive source.  However here is another list that has a few symbols that I don't think were shown in other notes, although there is a lot of duplication:

Dots 1-2-3-4-5-6:  Equal Sign (In another note this was shown as dots 1-2-4-5-6)
Dot 4: I have this as the Accent or Grave, but Judy could be right that it is the Tilde.  It should be noted, though, that when Judy says this needs to be capitalized to be treated as an at-sign, this does not mean preceding it with a dot 6.  Some note-takers require that the dot 4 be preceded with a Chord U before it.  As somebody else mentioned, sometimes note-takers require that the dot 4 be pressed with the backspace or dot 7 key to be accepted as an at-sign.  The at-sign is important, so you will need to find out how to enter it on the particular device you are using.

Dot 5: Quotation Mark
Dot 6: Comma

Dots 1-5-6: Colon
Dots 1-2-5-6:  Backslash
Dots 5-6: Semicolon
Dots 4-5:  Caret
Dots 4-5-6: Underscore
Dots 1-4-5-6: Question Mark
Dots 4-6: Period
Dots 1-4-6: Percent Sign
Dots 1-2-4-6:  Dollar Sign
Dots 1-4-5-6: Number Sign, (Not used to indicate numbers in this case but the actual character)
Dot 3: Apostrophe
Dots 1-2-3-4-6: the and-sign
Dots 2-4-6: Open Bracket (Sometimes is used for Open Brace)
Dots 1-2-4-5-6:  Closing Bracket (sometimes closing Brace)
Dots 3-4:  Slash
Dots 1-2-6:  Less-Than 
Dots 3-4-5:  Greater-than
Dots 2-3-4-6: Exclamation Point
Dots 1-2-3-5-6: Left Parenthesis
Dots 2-3-4-5-6:  Right Parenthesis
Dots 1-6: Asterisk

As noted elsewhere, numbers 1 through 9 and 0 are written as the letters a through I and j but in the lower portion of the cell.  In this code, numbers are not preceded by a number-sign.

If you are prompted to enter using computer braille, you should not use the 4-5-6 3-4-6 combination because the software is assuming that you will be entering computer braille.  You also do not need to terminate the computer braille with dots 4-5-6 and 1-5-6.  .

Jessie, computer braille is just uncontracted braille. For numbers, you use Nemeth code (lowered cells.) For instance, Nemeth 4 would be dots 2-5-6 (period), instead of number sign and d.
