I figured out to install the lanyard and got my lanyard installed a few
minutes ago.
There are thin string loops on each end of the lanyard for a good reason.
Take the reader out of the case and push each loop one at a time through
the lanyard points.  There's space under each point so the loop will go
through.
Once the first loop goes through, take the other end of the lanyard strap
and push it through the loop and pull it all the way through.  The loop
will wrap around that lanyard point.  Next push the loop through the other
lanyard point then detach the strap from that end by squeezing the arrows
together and hold the end in the point securely.  Next take the loose end
of the strap and push it through the string loop outside of the lanyard
point and then reconnect the strap to the plastic at the end of the second
lanyard loop.  If you then pull on the lanyard, the loop should tighten
around the second lanyard point.  Make sure the lanyard is flat before you
do this to help with wearing.  The buckle will be a bit difficult getting
through one of the string loops but it does fit.
