when i used to help take care of my grandma before she passed away, we used 
either desitin or boodros butt paste on her. they seemed to work the best.
some other things we used at times that i personally didn't think worked as 
well were vaseline, eucirin cream, a & d ointment, and even a triple 
antibiotic ointment at one time.
she was more than a bit incontinent though. she had many raw spots and such. 
she wore diapers that she wouldn't allow us to change more than a few times 
a day because of other medical problems that prevented her from breathing 
and such.
blah, sorry for the rambling, but i would go with desitin or the butt paste. 
i'm not sure i spelled it correctly though.
oh also, if the diaper rash or whatever you call it when it is on someone 
other than a baby gets persistently worse, the doctor can also prescribe 
silvideen cream, not sure i spelled that right either. also there is 
nistatin, again, not sure on the spelling of that one either, but the first 
i is supposed to be a long i sound.
