Speaking of doing something wrong.  The first time I took Gab to my mom to
watch her for a while, she actually put the diaper on backwards.  She had
never changed a paper diaper before and it seemed logical that the tabs
should be in the back so the baby could not reach them. Smile! V

And, the ability of the diaper to leak has little or nothing to do with
the skill of the diapererer. 
Just because it leaked, doesn't mean anybody did anything wrong.

Yes, they do.  Some brands worse than others.  I wouldn't take a box of
Luvs for free.
