Yes, I've experienced the same problem and have only carried my son's car
seat for very short distances.  Otherwise, I pull a stroller or in most
cases put him in a carrier on my back.

I would appreciate any suggestions on using a guide dog while carrying a car
seat. I have tried this and found it to be rather difficult. We do have a
stroller in to which the car seat can be placed and I have tried this with
moderate success. But if anyone has  other options I'd be interested in
hearing them.
