Hi,,
Yes this will work for the 4 as well as the 4s and 5.  Voiceover is apple's 
built-in screenreader and they have included it in all their phones, ipads 
and ipod touches from the 3gs up to the iphone 5 and everything in between.

The voice that you use on the iPhone is called voice over. Have someone go
to settings, general, accessibility and turn voice over on. Then, touch
anywhere on the screen and it will tell you what is there, and to open that
application you just double tap on it really fast.
