I myself, like to use a carrier. But when the toddler  get heavy this becomes  difficult. I do not own one myself but I have heard of several car seats that turn into strollers. I believe that there are several on the market. I do not know how they would do being pulled backwards so that your client could use her cane perhaps, she can go to a store and try them out.
