Dear Mainstream Customer,

The mission of the Central Ohio Transit Authority (COTA) is to provide quality customer service and meet the needs of the community.

Beginning on October 1, 2012, the new service provider for COTA's Will Call program will be UTS.

The purpose of the service is to provide rides for customers who receive ongoing, long-term medical treatment (such as dialysis, chemotherapy, and radiation therapy).

The Will Call program will operate Monday through Friday from

5:00 a.m. until 9:00 p.m., corresponding with the hours of operation for most medical facilities. Will Call service will not be offered on the weekends.

The cost for the Will Call service is $4.00 per one-way trip. The methods of payment are:

*        Cash

*        ADA ticket ($3.50) with a $.50 up charge

* COTA Mainstream ADA monthly pass with a $.50 up charge

Please note: Customers must have exact fare. Drivers do not carry change. In addition to paying the exact fare, the customer's COTA ADA identification must be shown to the driver.

Customers may call UTS directly to schedule a pickup and return trip to their medical treatment. In order to assist with receiving timely service, customers must request a trip at least one (1) hour prior to the scheduled medical appointment time. If a customer requires the use of a mobility device and/or requires having an approved Personal Care Attendant (PCA) ride with them on their trip, it should be specified during the reservations process. PCAs ride at no charge.

To participate in the Will Call program and to receive further information, please call COTA's Mobility Services information line at (614) 308-4300.

Sincerely,

Description:
