<p class="MsoNormal">Two key ways to know you have the right form 1) the new one expires on<span class="gmaildefault"><span style="font-family:"Tahoma",sans-serif"> </span></span>9/30/2027 and 2) visually the DOT logo has moved to be left justified<span class="gmaildefault"><span style="font-family:"Tahoma",sans-serif"> </span></span>whereas
before the logo was centered, so visually it's obvious if you have<span class="gmaildefault"><span style="font-family:"Tahoma",sans-serif"> </span></span>the old form or the current one.<br>

If the airline has a service animal form on their website then this PDF<br>
version is not needed, so it depends on the airline if you use this form or<br>
their website--check with the airline.<br>
