I had to do the same thing.if you're a diabetic there is no way that you can
fool this one.i don't get some of these insurance providers and the hoops we
have to jump through to get some of these supplies like insulin pumps and
cgms.

Those are also the Medicare requirements for getting my insulin pump
supplies covered by Medicare.  For the first time in 20 years, Medicare is
paying for my pump supplies.  So, I had to provide a 30 day log of my blood
sugars, proof that I am diabetic--my doctor laughed at this one.  I had to
do a C-peptide test along with blood work to prove that my pancreas does not
work, and I've been a diabetic for 50 years!  Since I have been on a pump
for 20 years, I already had the proof that I needed at least four injections
a day.  

Luckily, since I have been using a Dexcom, they could pull my readings from
it for the blood sugar log.  I am, however still waiting for my order to be
shipped.  They have not received the correct office notes from my doctor.
I've called three times now about my order, and I'm fortunate that the
Medtronic rep sent me a complimentary box of infusion sets.  I placed my
order on Jan. 11 and it is still being processed due to all of the Medicare
requirements.  


They assured me that it will not take this long in the future, since I will
have passed all of the Medicare requirements after this order is finally
approved for shipping.

  I'm on Medicare and am type 2.  I just went through getting my Dexcom not
that long ago.  I had to test my sugar 4 times a day for 30 days and keep a
chart to turn into my Diabetes educater.  I also had to be injecting 3 times
a day, which between insulin and Victoza I do.
And I believe she put in some of my really high numbers from a while back
when I wasn't so well controled.  I'm thinking that in general you might not
have to inject 3 times a day, but that it's one of Medicare's requirements.
Since I've had my Dexcom I've been going down on how much insulin I need.
It's definitely been helpful.
