It means that after the 12 hours, you will stop getting sensor readings. You can change the sensor anytime, of course, but once that 12 hours is up, the dexcom will completely die. You can end the session before the end of the 12 hours, but what I usually do is put on the new sensor a few hours before that, and just start the new sensor once the 12 hours is up.
