I use SuperAntiSpyware from http://www.superantispyware.com/. You have 
to run the free version manually, and I don't think it includes real 
time protection.

BTW, I think you sent this question to the wrong list. It seems off 
topic to me. I'd suggest the Blind Windows list for these types of 
questions.
