If this is a brand new iPhone 4S then you should be able to start
voiceover independently without using iTunes.
1) Plug your phone into an electrical outlet. This is necessary to
feel the vibrations described next.
2) Press and hold the power button on the top edge of the phone.
It'll be a single button along the short edge of the phone.
Hold the button for 3 seconds.
3) Wait until you feel the phone vibrate twice.
4) find the round button on the front face of the phone, near the
bottom. Press that button 3 times and you should hear "voiceover
on"
If this doesn't work, you can try setting up iTunes on your computer,
plugging your phone into the computer and seeing if iTunes recognizes
your phone. If it does, you'll be able to turn on voiceover from
within iTunes.
