Yes, a credit card is required.  Someone told me, that tips can now be 
added, but I haven't seen that entry yet.  Most people carry cash for tips.

You have 5 minutes to cancel your request.  If the request isn't canceled, 
then a $5 fee is charged.  However, if you can present a legitimate reason 
why you couldn't submit a cancellation, you are credited the $5 to your next 
ride.

I'm not sure what you mean by accommodations.  If you mean if there are 
accommodations for wheelchair users, yes.  You can ask for a van with a 
lift.

Let me note here.  Some of what I'm telling you may not be available in all 
cities.  I'm coming from a Phoenix perspective.

I must share this with you.  One of my coworkers was charged too much once, 
because the driver used her GPS to take her to her destination.  The GPS 
told her to use the freeway, instead of surface streets.  She said she was 
going to contact Uber about her trip.  She hasn't reported what happened as 
of yet.

Just remember.  If you treat others nicely, they will reciprocate.
