First TiVo’s do have accessibility features but remember you have to subscribe for monthly TiVo service or a lifetime service.

TiVo's accessibility features are available with TiVo Software Version 20.6.3 on:

* TiVo Mini

* They also say premier hd units MIGHT have accessibility

There is also a new voice remote. The voice remote works with TiVo BOLT, BOLT VOX, BOLT+ and Mini VOX
TiVo Roamio DVRs (Pro, Plus and OTA) and TiVo Mini when combined with Bluetooth® dongle (included)
TiVo VOX Remote does NOT work with any prior models of TiVo DVRs.
If you use cable you’ll need a cable card in your TiVo to get service. Normally you can get one free cable card from the cable company.
There are other TTS options now too.
Like mentioned before you have Apple TV, along with streaming it also allows you to access most channels you subscribe to they cable with each channels individual app.
Use also have Comcast DVR, fios DVR, and both direct tv and dish have tts screen reader options.
If you go Apple TV there is also something new I’ve found called channel dvr which allows you to access any tunable channel thru your Apple TV, you will need a cable card if you use it with cable or cut cable and use a antenna. Your also need a network tuner called a HDHomeRun.
The channels app for Apple TV and iOS is voice over compatible also.
