It signals the app directly, but I am slow thus far at clicking on the
correct response or choice.  I think that is just because I am not used to
it.

Yes, it will not allow two way communication without the echo spot or the
echo show.  It will however connect to a echo dot to alert you when someone
is at the door and you can see the video on your phone and talk to the
person through your phone app.
