I am thinking the problem lies with monkey survey dot com. Not sure if
there is anything you can do without the assistance of an assistive
technologies team guiding the web master at monkey survey. Get me their
corporate office info and I could help in that regard..

Hi Penny and Albert,

Thank you very much for taking the time to participate in my survey.
Thank you also for your feedback regarding accessibility issues and
suggestions for answer choices.

I will email the survey to the group.

Penny, can you please tell me which screen reader you use so that I
can make improvement next time?

Thank you very much.

Best,
Phil
