Andrew,
Just take a deep breath, calm down and pray. Words will come to you. Just 
explain your situation to her after you introduce yourself.
I'd say a great step to your growth and being active in church and life is 
learning braille.
I use large print and braille. I  was a scripture reader at church for a 
while. I also was in a few church plays as a kid, but I used braille to 
memorize my lines. Also, I read the hymnal in braille. All these things 
helped me participate in church like everyone else.
