Job Summary:  Under the supervision of the Associate Director and as a
member of the Association's leadership team, the Adult Rehabilitation
Services Director is responsible for helping the organization achieve its
mission of inspiring all adults to achieve their greatest potential. Key
responsibilities include the implementation and service delivery of all
Adult Rehabilitation programs.  This includes financial management and
department budgets, human resource management and allocations, business
development and collaboration, service delivery of programs, strategic
planning, marketing, operations management, and quality control management. 

 

Performance Standards and Expectations

 

1. Oversee the day-to-day operations of Adult Rehabilitation Services to
ensure all clients receive quality services.  This includes intake and
referrals, monitoring client outcome measurement and performance toward
goals, customer service, management of staff, budgeting, prepare report(s)
for internal and external audiences, and billing management.

 

2. Develop goals and objectives for Adult Rehabilitation programs based upon
the strategic focus of the organization and monitors progress toward
achieving those goals.

 

3. Responsible for conducting ongoing program evaluation, collecting,
analyzing and interpreting data. Identify needs and establish program
priorities, communicates information to appropriate staff.

 

4. Actively develops existing and new programs in the Adult Rehabilitation
area. Examine program effectiveness to strategically explore, identify and
recommend new program directions, possible program models, and service
delivery.

 

5. Analyzes the direct service staffing needs and presents information and
recommendations to the Associate Director concerning these needs. 

 

6. Develops and maintains a team work approach to providing services.
Manages and develops staff by measuring performance, providing feedback and
direction, and opportunities for professional development.  

 

7. Hires and dismisses direct service staff following review and direction
from the Human Resources Director/Associate Director.

 

8. Works closely with the Finance/Associate Director to develop program
budgets and monitors program revenue and expenses. This includes strategies
and processes to ensure and maximize third party billing. 

 

9. Develops and maintains strong working relationships and partnerships with
referral sources, i.e. eye care professionals, Milwaukee County, Department
of Vocational Rehabilitation, Office for the Blind and Visually Impaired,
and others.  Develops new relationships, collaboration and business
opportunities. 

 

10. Serves as the contact person for contracts relating to the Adult
Rehabilitations Services program at Vision Forward and establishes contracts
with funders, vendors and referral sources. Ensure contractual obligations
and compliance requirements are met.  

 

11. Reviews carefully all important or contractual paperwork completed in
the department, including all required record keeping to minimize problems
or potential liabilities.

 

12. Develops and maintains good working relationships with other internal
departments as a contribution toward a cooperative, coordinated and
similarly focused organization.

 

13. Work closely with the Marketing Director to develop and implement
marketing strategies to increase awareness and referrals for Adult
Rehabilitations Services programs.  

 

14. Actively participates in community outreach efforts.  Facilitates
presentations and serves on committees and councils that can help fulfill
the organization's mission. 

 

15. Actively participates on the Association's leadership team and works
with other managers and staff to enhance the organization's mission, goals,
and teamwork. Supports integrated programs and service delivery.

 

16. Works closely with the Volunteer Coordinator regarding volunteer
opportunities, recruitment, training and supervision of volunteers who work
with direct staff and program participants.

 

17. Maintain a professional environment conducive to positive relationships
among clients, family members/caretakers, program staff, department
personnel, volunteers, DVR, partners and the general public.  

 

18. Examine and follow trends related to vision loss and programming for
adults by maintaining contact with organizations offering services to adults
who are blind or visually impaired. Research and review current literature.

 

19.  Work with the Executive Director and leadership team to communicate the
mission and vision of the Association and participates in the development,
implementation and review of the strategic plan.

 

20. Other duties as assigned.

Job Summary:  Under the broad supervision of the Adult Rehabilitation
Services Director, the CVRT will provide assessment and training to a wide
range of clients with visual impairment.  Areas of focus will include
activities of daily living, expressive and receptive communications enabling
adults who are blind or have low vision to confidently carry out a range of
daily activities. The CVRT will work closely as a member of a
multidisciplinary team that includes occupational therapists, orientation
and mobility, optometrist and technology instruction to strategically
implement and expand the goals and objectives of the program.  Primary
service delivery will focus on adults, but may include some work with school
age children. 

 

Performance Standards and Expectations

 

1. Assess and evaluate clients' ADL and expressive and receptive
communication skills.  Work with clients to establish specific goals,
strategies and plans for attaining goals.  Recommend appropriate equipment
for meeting those goals. Provide re-evaluation as needed.

 

2. Identify visual impairments related to basic life skills in areas such as
self-care, literacy, communication, health management, home management, and
meal preparation.

 

3. Provides vision rehabilitation therapy regarding communication, including
Braille; Personal Management such as grooming, hygiene, clothing
organization, and identification; Activities of Daily Living such as
cooking, cleaning, shopping, safety, money organization and management;
leisure and recreation activities and Adjustment to Blindness training.

 

4. Provides vision rehabilitation therapy in various environments focused on
assisting the client in achieving their goals.  Monitors client progress
toward goals and adjusts training as needed.  Therapy may be delivered
one-to-one or in groups.

 

5. Educates clients and family members about their particular diagnosis,
functional implications, potential interventions, benefits of vision
rehabilitation, and other resources within and outside the Association. 

 

6. Monitors clients progress with regard to established goals and document
services delivered.

 

7. Consult and work with team members of multi-disciplinary teams to
incorporate appropriate strategies and goals into the clients' plan, to
ensure the clients' fullest development and progress.

 

8. Completes appropriate documentation, billing, and other reporting
functions in a timely and accurate manner.  Provides appropriate
correspondence for referral sources, like eye care professionals and DVR.
Maintains up-to-date accurate records and completes accurate entries into
database.  Maintains assigned monthly and annual reports. 

 

9. Actively works and participates in collaboration with members of the
Adult Rehabilitation Services team.  Develops and maintains a team work
approach to providing services. Makes appropriate cross referrals.  Actively
participates in team meetings and activities. 

 

10. Serves as a resource for clients, families, team members, professionals,
and the general community in vision rehabilitation therapy.  Presents
information to groups, participates in health fairs, and markets to referral
sources.   

 

11. Assists in the development of new programs/service delivery models and
works to upgrade existing ones. 

 

12. Remains current in practices and techniques related to vision
rehabilitation therapy, knowledge of new techniques and practices,
understand medical aspects of vision loss, and devices available for people
with vision loss.

 

13. Attend in-service trainings, professional workshops, conferences and
seminars to continually enhance professional skills and knowledge.

 

14. Contributes to the functioning of the organization by attending staff
meetings, participating in discussion and accepting committee assignments to
complete various tasks related to achieving the mission and goals of the
organization.
