Dear Community Members,

I am writing today to ask that you complete the survey below. We thank you in advance for your participation and support on an important project in the Baltimore Office of Sustainability (Bos)! The BoS is currently working on updating the Disaster Preparedness and Planning Project<https://www.baltimoresustainability.org/plans/disaster-preparedness-plan/> (DP3).

In an effort to include community perspectives and use an equity lens in this plan update, an Equity and Outreach subcommittee was created. One way the Equity and Outreach subcommittee is gathering community perspectives is through a survey aimed to better understand the concerns and needs of different communities regarding potential hazards and threats. The goal is to spread this survey to as many people in Baltimore City as possible by next Thursday, July 26th, and that is why your help is so valuable.

If you could, we ask that you please:

  * Share this survey with your email distributions and networks - Baltimore Community Perspectives on Risk<https://www.surveymonkey.com/r/KZKRPCW>. (Full link can be found here: https://www.surveymonkey.com/r/KZKRPCW<https://www.surveymonkey.com/r/KZKRPCW>).
     * A hard copy of the survey is also attached. Please feel free to use this method and support staff can pick up the responses or receive scanned answers.
     * Utilize any organizational networks you may have to allow the survey to reach all the communities with which you are connected.
  * Send the link to your associates, friends, and family in the city!

Please note that this survey is completely anonymous, and participants may opt out of providing any personal information.

We hope that you are generous enough to lend your energy and time to this project, and we appreciate and welcome any feedback or comments! For more information, please visit the Office of Sustainability website<https://www.baltimoresustainability.org/> or contact Healthcare Ready<http://www.healthcareready.org/>, a non-profit supporting the Equity and Outreach team, at contactus at healthcareready.org<mailto:contactus at healthcareready.org>.

Best,
Crystal
