[Greater-Baltimore] Mobility Paratransit Form Not accessible

Hi Elizabeth,
Recently, Marian Gushtala and Mary Schoch from the Greater Baltimore Chapter advocated so that MTA would make the MTA Mobility certification and recertification forms accessible.  They are attached.  I believe MTA is in the process of posting and distributing the accessible versions, but given that it takes time to socialize new versions, I'm not surprised you have a version that doesn't work well.

Hi All,

I'm new to this area and I'm filling out the Mobility form as a PDf.
Is there a way to work with them to make these forms more accessible, such as filling them out on line? I have been returning to my PDF and realized that I have accidentally wrote my information over boxes that my Healthcare Provider also filled out. The provider who filled out their portion of the form is blind and also had a hard time filling it out. They also accidentally wrote their information in my section of the application and I'm having to fix it.
How can we change this?
Could we perhaps try and create a separate downloadable Pdf that we can send to our provider, so they can fill out their portion of the application?
