<span style="font-family:"Arial",sans-serif">An interview (1-1.5 hours). You can complete the interview remotely via phone call or video call.<o:p></o:p></span></li></ol>

<span style="font-family:"Arial",sans-serif">Have a diagnosis of macular degeneration OR glaucoma for<em><span style="font-family:"Arial",sans-serif"> at least 10 years</span></em><i><o:p></o:p></i></span></li><li class="MsoListParagraphCxSpMiddle" style="margin-left:0in;mso-add-space:auto;mso-list:l0 level1 lfo2">
<span style="font-family:"Arial",sans-serif">Have serious difficulty seeing even when wearing glasses or contact lenses (or unable to see)<o:p></o:p></span></li><li class="MsoListParagraphCxSpMiddle" style="margin-left:0in;mso-add-space:auto;mso-list:l0 level1 lfo2">
<span style="font-family:"Arial",sans-serif">Have vision loss that affects daily living activities (e.g., reading, cooking, and walking)<o:p></o:p></span></li><li class="MsoListParagraphCxSpLast" style="margin-left:0in;mso-add-space:auto;mso-list:l0 level1 lfo2">
