[gui-talk] Book on CD

Sorry about the cross post.  I have had my autobiography "Suddenly Blind" (available on Amazon and soon Kindle)  published and am looking to put it on CD.  I am looking for answers for who does this, who might know what type of equipment / software that I would need or someone has to do this?  I will appreciate any advice / suggestions.
Of course the cd will need to have page markers, chapter markings...
