Aloha,

Attached is a questionnaire for anyone blind or print disabled, regarding
your accessible voting experience in the 2020 Hawai'i primary election. Even
if you did not vote, you can fill it out and return it.
We are compiling the data to work with our office of elections and
legislators to improve future accessible voting experiences for our blind
and print disabled citizens of Hawaii.

Please fill the questionnaire out and return it to me no later than
September 30, 2020. If you cannot fill the form out, please contact me and
we will arrange to have someone call you to assist in completing the
questionnaire.

Please feel free to pass it on to others in Hawaii who are blind and print
disabled. They do not have to be registered to vote to complete the
questionnaire.

Hope most of you voted in the primary and if not get, registered to vote
November 3 in the general election!
