Hi Danielle,
Do you have an eye report? If so, you should be able to submit 
that.  I have not had an eye exam since high school and they took 
my eye report with no problem.  My vision does not change, so as 
long as your condition is not degenerative, you should be fine 
submitting your original eye report.
Another option would be if you have an nls membership.  I'm not 
sure about Hadley, but bookshare lets you submit your information 
you have on file with nls for verification of disability.  
Something to look into.
