Ashley gave some good advice for your specific issues that you mentioned. I 
just wanted to add that for the hardcopy forms there is a way for you to 
recreate those forms electronically and you are able to check boxes and fill 
in the information where it is necessary. I have an old document that states 
how to do this in older versions of word, I don't know what version of word 
you use, but I'm sure you can also find instructions online as to how to 
create accessible forms in word using the current version you have.
