Intake and Resource Coordinator Job Description

Under the supervision of the director of social services, using the social
model of disability and peer empowerment principles, the intake and resource
coordinator will:

*        Provide a welcoming introduction to MABVI to potential program
participants, families, and the public;

*        Conduct telephone interviews with potential participants, including
sensitive and careful assessment of individual needs and motivation for
services;

*        Provide intra-agency and interagency information and referral
services as needed;

*        Respond to inquiries from family members, service providers, and
the public about agency services and programs;

*        Augment and maintain current database of local and national
blindness and multi-disability resources;

*        Assist in the development of forms that will be used to refer
consumers to MABVI services;

*        Maintain records of all intakes and work with leadership to
identify trends in participant responses to intra-agency referrals;

*        Help to identify resources to serve diverse communities and
under-served populations within the disability community;

*        Work with program staff and leadership to identify gaps in service
needs;

*        Participate in the development of new initiatives with program
staff;

*        Follow all confidentiality laws and HIPAA digital compliance
standards;

*        Assist in upkeep of electronic healthcare records database and case
management processes;

*        Participate in outreach or community engagement as needed.

 

Qualifications:

Bachelor's degree in social work or related field, strong interpersonal and
communication skills, proficiency in Windows and Microsoft platforms,
multi-lingual proficiency a plus, lived experience with visual disability
strongly preferred.

Excellent follow-through, teamwork  and collaboration, stress management of
escalating situations, and proficiency with healthcare sector processes and
referrals preferred.
