[HumanSer] Documentation needed for LMSW exam accommodations

Hi Everyone,
My ophthalmologist and I have completed the non-standard testing arrangements accommodations form. For those of you who have taken the LMSW exam recently and have submitted accommodation request, do you know if any other documentation is required to be submitted? For instance, do I need to compose and submit an additional pe personal statement? I have no other reasons for accommodation beyond my blindness. I have emailed special arrangements, but I wasn’t sure if anyone here could also provide feedback.
