I got everything sorted out by calling the RTA and completi8ng the task over the phone. I wish the RAP page was a bit more clear about where to find the correct form to request additional rides. Oh well, at least I got the task done.

Indeed this does not appear to be related to the CTA Accessibility Survey. However this is also not feedback that is solely related to applying for the additional ten rides.
So I do just want to ensure that everyone understands that if you are a current, registered TAP/RAP user you will not need to upload any form of ID or any other document to qualify for the increase.
ID may be needed for other services available through the current transit system, such as applying for Paratransit but the ten additional rides only concerns those who are already certified and therefore no documentation is needed.
Just to ensure it’s known, the new transit authority does not yet exist and won’t really exist until next year. The only new addition to the RTA’s website that we’ve been made aware of is this ability to apply for ten rides.
That being said any and all accessibility concerns are valid and I highly encourage feedback to be sent to both us and whichever transit entity you are specifically referencing but please try to ensure it’s under the related email thread. I will make note of these concerns but please direct any further concerns related to RTA/PACE/Paratransit either under my thread regarding applying for the ten rides or to me directly at kobradov at gmail.com <mailto:kobradov at gmail.com> so that our advocacy committee can be made aware.
Hope all Chicagoans using the CTA will participate in this accessibility survey, while we were glad to have some success with the braille and large print bus stop signs as riders we know there is more that needs to be improved! (And thank you, Sara, for your continued representation and advocacy on the CTA’s ADA Advisory committee and for sharing this survey!)

That form is completely unrelated to the survey, my email was referring to That, was sharing a survey from CTA to get feedback from individuals with disabilities on any accessibility barriers, they face while riding the CTA bus or train. I’m sorry to hear that you had accessibility challenges with the RTA form but, if you are interested in filling out the CTA form, I can confirm that it is completely accessible and, you do not have to provide any personal information to submit this survey.

I tried filling out the form and ran into some accessibility and usability issues. The birthdate drop-down menus are not adequately labeled. The form wouldn’t accept pictures of my state id even though they are valid. There is a field labeled ‘center’ which doesn’t mean anything. The form instructions are inadequate. On the last page I saw a list of services I could potentially apply to even though I am already certified. I needed to use the select buttons for each item like a doctor’s letter, proof of SSDI or Medicare letter, and selecting an option for free public transportation. I got to the point where I uploaded my state id but that wasn’t adequate for the form. I guess this new transit authority wants to make it as difficult as possible for people to apply for additional rides. I wonder if or who tested this form for accessibility. IU don’t like the way it is designed.
