Subject: IL Voter Polling Place Accessibility Survey

Hello all.  In an effort to identify polling place accessibility barriers
across the State, EFE is asking voters with disabilities to complete and
return the attached survey when they go to vote on November 2, 2010.  It's
attached in two different versions of Word.  Please let me know if you are
unable to access either document.  

EFE will provide hard copies of the survey and postage pre-paid return
envelopes upon request.  The survey will also soon be posted on EFE's
website, www.equipforequality.org. 

Thank you all for your help with this effort to identify polling place
accessibility problems.
