I tried to access the form on my iPhone, and it didn't work that way, either. The next best thing is to copy the letter and send it individually to those who are designated.

The form appears to be inaccessible with screen readers. Specifically, I cannot find a way to fill in my name or address; neither JAWS nor NVDA recognizes those fields as edit boxes.

Are you aware of any workaround other than seeking sighted assistance?
