I don't know of a street guide app, but I recently found a PDF listing all
of the Chicago streets and their number in the city grid. You can download
it and store it for future reference.

Ideally, there'd be an app that would allow you to put in the name of a
street and get the numeric location; e.g., you'd put in Western Ave. and
it'd tell you that it was 2400 west.  Please let me know if you're aware of
an app that has that functionality.
