[IL-Talk] Fw: 2025 Chicago Chapter Bowling Fundraiser Entry Form

Hello Everyone,

When I attempted to complete the bowling form using the link on my phone, I ran into a problem.  Once I filled out everything and clicked on the “review” button, everything disappeared and I got a new blank form as though I had never filled it out.  So I never got to make a payment.
