The birthdate drop-down menus are not adequately labeled. The form wouldn\u2019t accept pictures of my state id even though they are valid. There is a field labeled \u2018center\u2019 which doesn\u2019t mean anything. The form instructions are inadequate. On the last page I saw a list of services I could potentially apply to even though I am already certified. I needed to use the select buttons for each item like a doctor\u2019s letter, proof of SSDI or Medicare letter, and selecting an option for free public transportation. I got to the point where I uploaded my state id but that wasn\u2019t adequate for the form. I guess this new transit authority wants to make it as difficult as possible for people to apply for additional rides. I wonder if or who tested this form for accessibility.
